Unsupervised worker blamed for overriding alarms in Michigan chemical spill
A Tribar Manufacturing wastewater operator overrode waste treatment alarms 460 times during the July 29 incident and failed to immediately notify Michigan authorities after discovering the discharge, according to the violation report released Aug. 10 by Michigan Department of Environment Great Lakes and Energy (EGLE).
